Location: We are within the Midvale Middle School at 7852 South Pioneer Street (310 West). We are in the south west building on the school grounds, next to the track field.

Hours: 8:00 a.m.- 4:00 p.m. Monday-Thursday. We are closed Fridays.
Walk-in times (at these times, no appointment is needed for children 18 or younger who are sick): 8:30 a.m.- 9:00 a.m. 1:00 p.m.- 1:30 p.m.

Closed: 12:00 p.m.- 1:00 p.m. for lunch

Services Offered

Evaluation and treatment of health problems such as colds, infections, headaches, rashes, minor injuries.
Treatment for long-term health problems such as diabetes, high blood pressure, arthritis, and asthma.
Physical exams for infants, children and adults.
Immunizations
Assistance in finding affordable or free specialists, and help with medical-related bills.
Help with obtaining affordable or free prescriptions.
Referrels to other social service programs such as WIC, Food Shamps, Medicaid, and CHIP.

About Us

All our staff members are bilingual (Spanish-English). The full-time Medical Provider and Clinic Director is Kim Spangrude, Family Nurse Practitioner. In addition, our bilingual Medical Assistant serves as translator for Spanish-speaking patients. The Front-Desk Coordinator is available by phone from 8 AM to 4 PM Monday through Thursday (except during lunch hour) and our Americorp Volunteer joins us as the Medical Resource Coordinator, locating resources such as free medications, specialty consults, out-of-clinic tests such as x-rays, etc., and social services.

Children under 18 may be seen without their patents if the patents have signed a "permission to treat" form (will be available in the Fall of 2006).
